Our Outpatient Billing Process

Yes, I have Insurance

When all testing has been completed, WaterforDx will le a claim for all lab services rendered on the date of your office visit.
You should receive an explanation of benefits (EOB) from your insurance company. An EOB is not a bill. It serves to explain the services you received during your office visit.

When WaterforDx files a claim and a balance is due, you may receive a bill. WaterforDx has extensive relationships with a large number of insurance networks, offering flexible payment options that will best meet your needs.

No, I don’t have Insurance

Our billing department features highly trained lab specialists who will work one-on-one with you to determine what flexible payment plans are available to you.

In Network with most major insurance carriers.

If the EOB states that WaterforDx is an out-of-network provider, then we will make all attempts possible to ensure the patient is only responsible for the amount they would owe if WaterforDx were an in-network provider, to the extent permissible by law and according to any contractual obligations.

*if you have any questions about a bill or EOB that you have received, please do not hesitate to call our patient call center at 623-280-9926. A friendly call center representative will be happy to assist with your billing questions.
